If you have not done so already, please complete the mandatory CPAF questionnaire.

As part of the submission, you will need to check that all of your Pharmacists and Pharmacy Technicians have completed Level 1 and 2 Safeguarding training. You can access this via the NHSE E-learning for Healthcare website.
Information collected from this questionnaire helps to identify whether a pharmacy needs visiting. If a pharmacy is considered for a visit, the full, comprehensive CPAF pre-visit questionnaire will be initiated.
As there are significant pressures in Community Pharmacy at this time, please complete this form as soon as possible to avoid the need to complete the full CPAF and also receive assurance visits from NHSE which will add additional workload and burden to you and your teams.
