Last year, your views about our website and newsletter helped improve the effectiveness of our communications, provide you with concise newsletter updates and revamp our website, making it more user-friendly.

We also want to raise awareness of the great work you’re all doing in your local communities. If you’d like to share any success stories, please email karishma@cpgm.org.uk

Every entry will be entered into the prize draw for the chance to win one of 8 x £25 vouchers. Full terms and conditions are listed below. By completing the survey, you are confirming that you accept the below conditions in relation to the prize draw.

1. This prize draw is open to all members of the pharmacy team and also to locum pharmacists and pharmacy technicians who work in pharmacies located in Community Pharmacy Greater Manchester.
2. All members of the pharmacy team can provide feedback and enter the prize draw.
3. Each person can only enter the prize draw once. Multiple entries from the same person will be removed.
4. You will need to provide your contractor ODS for verification.
5. Completing the survey is optional.
6. Entering the prize draw is optional.
7. You can provide feedback anonymously however you will not be placed into the prize draw.
8. To enter the prize draw you must provide your name and contact details. Without this information you will not be placed in the draw.
9. The prizes are 8 x £25 Love to Shop vouchers. No other alternatives will be provided.
10. The closing date for the prize draw is 12pm noon on 28^th June 2024
11. The prize draw will be made using an automated system on 1^st July 2024.
12. The survey may remain open for feedback after this date however you will not be entered into the prize draw.
13. Winners will be contacted after 1^st July 2024
14. A list of winners will be available from 1st July 2024 by contacting enquiries@cpgm.org.uk
15. The Chair’s decision is final.
