DMS: Activity Data and How to Maximise the Opportunity

Discharge Medicines Service: Are you maximising the opportunity? 

Following feedback from contractors at recent CPGM Connect events highlighting the need for greater access to data and insights, we are pleased to share the latest Discharge Medicines Service (DMS) activity across Greater Manchester.

Community pharmacies delivered an impressive 18,156 Discharge Medicines Service (DMS) interventions in 2025, generating £523,930 in income. This achievement reflects the continued dedication of community pharmacy teams.

With pharmacies able to claim up to £35 per completed referral, the service also represents a significant source of income across our localities.

CPGM are committed to working with Trusts across Greater Manchester to expand referral activity and support further integration, ensuring more patients can benefit from DMS and that pharmacies are able to maximise this opportunity.

DMS continues to offer a significant opportunity for both improving patient outcomes and increasing pharmacy income.

Pharmacies can further enhance outcomes by:

• Increasing conversion of referrals into full completions
• Strengthening follow-up to support progression to Stage 3
• Embedding DMS into routine workflow
