PQS: Palliative and End of Life Care – Complete the form by 28th October

We are collating local information to help Greater Manchester pharmacies complete their PQS Palliative and End of Life Care action plan.

Please complete this short form by 28th October even if your pharmacy does not hold, or intend to hold, these medicines. We would like a response from every pharmacy.

The form asks if your pharmacy:

• Routinely stocks the 16 PEoLC medicines
• Can support local access to parenteral haloperidol
• ls currently commissioned to provide a local palliative and end of life care service

We are using information about currently commissioned palliative care service providers whilst we wait for NHS GM’s announcement of the updated service. This is the best information currently available and will be updated once the new arrangements are confirmed.
