CPAF 2026/27: Screening Questionnaire – Complete By 31st July
The Community Pharmacy Assurance Framework (CPAF) 2026/27 screening questionnaire is now open and must be completed by 11:59pm on Friday 31st July 2026. Completion of the screening questionnaire is a mandatory Terms of Service requirement for all community pharmacy contractors in England.
Alongside the launch of the screening questionnaire, the NHSBSA has published the 2026/27 full CPAF questionnaire. Although only a proportion of pharmacies will be selected to complete the full questionnaire later in the year, all contractors are encouraged to review it now as a valuable self-assessment and compliance tool.
Why is this important?
The screening questionnaire consists of 10 questions and the responses, together with other information available to NHS England and Integrated Care Boards (ICBs), will be used to determine which pharmacies

May be selected for a contract monitoring visit; and/or

Will be invited to complete the full CPAF questionnaire later in the year
The published full questionnaire provides a clear indication of the evidence that may be requested during the assurance process and can help contractors identify any areas requiring attention before they are selected for further assurance.
Key actions for contractors
- Complete the CPAF screening questionnaire before 31 July 2026. A copy can be downloaded here
- Review the published full CPAF questionnaire and use it as a self-assessment tool
- Check that policies, SOPs and mandatory training records are current and accessible
- Ensure evidence is available to demonstrate compliance with the Community Pharmacy Contractual Framework (CPCF), including service delivery, governance and record keeping
- Address any gaps identified now rather than waiting until the full CPAF process or a contract monitoring visit
